If you can see it in W3D viewer it should be ok in the game or in worldbuilder.
Check that you did not spell the name of the model wrong in the ini code.
Also, you cannot rename W3D files using the operating system, so if you renamed it, rename it back to what it was before, or else re-export it from Gmax to the filename you want.
Also check the size of the model, i once heard of a guy who made his model too big so he could not see it in the game because the camera was inside the model.
And if it uses a skeleton (as in infantry) make sure the skeleton file is also present.
